How to Apply 

  • Online Directions:
  1. Read the applications carefully and fill out all applicable fields.
  2. Attach required documents. Pictures of documents will not be accepted.
  3. Once all fields have been filled out, a "Submit" button will show on the bottom.
  4. Press submit and enter a valid email.
  5. Follow the directions given on the next screen: go to your email, look for an email from "Adobe Sign", and click on "Confirm my email address"
  6. The completed form will be emailed to you for your records.
  7. If needed, SJVA office staff will reach out to you for additional documentation.
 
We strongly recommend submitting your student’s application online. However, if a paper application is needed, please download and print from the PDFs available below.

TK AND KINDERGARTEN

  • Age Requirements:
    • Students entering Transitional Kindergarten must turn 4 years old by September 1st, 2026.
    • Students entering Kindergarten must turn 5 years old by September 1st, 2026. 
  • Applications are available in English and Spanish
  • Required documents 
    • Copy of Birth Certificate (In English - From any Country)
    • Immunization Record (From any Country, Yellow Card or Doctor Printout – Not from another school)
    • Health Examination Form (Dated AFTER February 13th, 2026)
    • Oral Health Assessment Form 
    • Expulsion Affidavit (if applicable)
    • Valid Government Issued ID of BOTH Parents or Guardians
    • Caregiver Affidavit (if applicable)
    • Expulsion Affidavit (if applicable)
  • Waivers for the oral and health examinations are available at the main school office or below
